Patient safety: what is your risk?

Transitions of care involve movement of patients within the same healthcare facility or across different facilities, involving consultations with different healthcare providers.

An individual patient’s risk of experiencing medication-related harm increases with the number of medications (polypharmacy), high-risk medications or in the presence of morbidities, such as kidney impairment.

Medication discrepancy refers to any intentional or unintentional difference between your medication use history and the medication orders. Unintentional medication discrepancy could take place when patients receive multiple medications from different prescribers across different healthcare facilities.
